Tier 2 Event Medical Support

Recommendations for Tier 2 Event Medical Support

Let’s scale things up a little shall we?

Tier 2 events bridge the gap between small community gatherings and major festivals. While they don’t always require a full fleet of ambulances, the increased headcount and complexity mean that a “kit and a volunteer” approach is usually no longer adequate or compliant.

Tier 2 Event Medical Support

What Defines a Tier 2 Event Medical Support?

Typically, an event moves into Tier 2 if it meets several of the following criteria:

  • Attendance: Between 500 and 2,000 people.

  • Duration: Full-day events or multi-day programs.

  • Risk Profile: Includes moderate-risk activities, alcohol sales, or a diverse age demographic (e.g., seniors or young children).

  • Environment: Large outdoor spaces or venues where the “nearest A&E” isn’t immediately accessible.

Recommended Level of Event Medical Support

According to the Purple Guide, Tier 2 events often require a formalised medical presence. While you may not need a doctor on-site, you should move beyond basic first aid to include:

  • Tier 2: These events are somewhat larger events which often fall within the scope of local authority licensing.  The presence of some of these indicates that event medical support may be in Tier 2:
    • duration more than a few hours but no longer than a day
    • low risk of illness or injury from activities
    • social drinking of alcohol
    • no more than isolated drug use
    • up to two thousand attendees
    • hospital referrals unlikely

    Cover level
Tier 2 events need a dedicated first aid resource, preferably led by a healthcare professional. Where indicated by the medical needs assessment, provision may include:

    • a nominated lead who is responsible for the delivery of the service on site
    • supporting first responders or healthcare professionals
    • an ambulance with suitably qualified crew if hospital transfers are expected
  • Static First Aid Post: A visible, central location for treatment.

  • Advanced Equipment: On-site AEDs (defibrillators) and trauma kits.

  • Communication Plan: A robust system for alerting medical staff without relying on public 999 services for minor incidents.#

  • REMEMBER: The calculation of medical needs should not be based solely on expected numbers attending but should take account of all the relevant factors.

Determining Your Event Medical Support Needs

The leap from Tier 1 to Tier 2 is often dictated by your Medical Risk Assessment. We evaluate five key pillars:

  1. Audience Profile: Does the crowd involve high-energy activity, alcohol consumption, or vulnerable groups?

  2. Activity Level: Are there hazardous elements, such as motorized displays, contact sports, or pyrotechnics?

  3. Venue Logistics: Is the site sprawling? Can an NHS ambulance easily access the heart of the event if needed?

  4. Weather & Timing: Will heatwaves, cold, or darkness increase the likelihood of trips, falls, or exhaustion?

  5. NHS Impact: Will your event put undue pressure on local primary care? A Tier 2 provider’s job is to treat on-site to avoid unnecessary hospital transfers.

Key Steps for Organisers

  • Professional Consultation: Engage with a competent provider to review your risk assessment. For Tier 2, your local Safety Advisory Group (SAG) will likely want to see a formal Medical Plan.

  • Check Credentials: Ensure your provider carries specific Public Liability Insurance. If they are providing Paramedics or Nurses, they must be registered with the HCPC or NMC.

  • CQC Compliance: If the provider suggests they can transport patients to the hospital or perform “Treatment of Disease, Disorder, or Injury,” they must be CQC-registered. Be wary of providers offering “free” ambulance transport without the proper legal registration.

The RTC Difference: At RTC Medical Solutions, we don’t believe in “upselling” Tier 2 events into Tier 3 budgets. Our focus is on proportionate cover. We provide the clinical excellence you need to stay compliant and safe, ensuring that your medical provision is a seamless, professional safety net—not an overpriced branding exercise.
